For the historical catastrophic threshold, the published amount is $8,000. The pre-2025 catastrophic threshold was not a hard out-of-pocket cap.

Which income counts for IRMAA?
Modified adjusted gross income is ordinarily adjusted gross income plus tax-exempt interest from two tax years earlier. Certain changes can justify a new determination from SSA.
Will a married couple pay one premium?
Medicare costs are generally per enrolled person. Joint filing uses the joint income thresholds, but each spouse can owe a premium and surcharge.
Does the Part D base amount equal my plan premium?
No. It is a national benchmark. Your chosen plan premium and any Part D income-related surcharge are separate.
Can retirement reduce an IRMAA assessment?
Work stoppage or reduction can be a qualifying life-changing event. Follow SSA-44 and SSA’s evidence rules; a lower entered calculator income does not itself change the assessment.
