2028 Medicare Part D Base Premium: $43.81 (Projected)
Planning projections. The final CMS amounts and income tiers can differ.
Projected
Part D national base beneficiary premium
$43.81
ProjectedThis is a benchmark, not your bill.
Your plan sets your actual monthly premium. The base amount is used in penalty calculations and other Medicare formulas. The maximum standard deductible is $685.
For covered Part D drug out-of-pocket spending, the projected amount is $2,350. Premiums and non-covered medicines do not count toward that drug-spending cap.

Questions, answered.
Are the 2028 Medicare amounts official?
No. These are Trustees-based premium projections and explicit IRMAA planning assumptions, with no claim of a final CMS announcement.
Which income counts for IRMAA?
Modified adjusted gross income is ordinarily adjusted gross income plus tax-exempt interest from two tax years earlier. Certain changes can justify a new determination from SSA.
Will a married couple pay one premium?
Medicare costs are generally per enrolled person. Joint filing uses the joint income thresholds, but each spouse can owe a premium and surcharge.
Does the Part D base amount equal my plan premium?
No. It is a national benchmark. Your chosen plan premium and any Part D income-related surcharge are separate.
Can retirement reduce an IRMAA assessment?
Work stoppage or reduction can be a qualifying life-changing event. Follow SSA-44 and SSA’s evidence rules; a lower entered calculator income does not itself change the assessment.
