2028 IRMAA Brackets: Married Separately from $224.50 (Projected)
Planning projections. The final CMS amounts and income tiers can differ.
Projected
Coverage year 2028 ordinarily uses the 2026 tax return. Amounts below are per enrolled person.
| MAGI from 2026 | Part B total / month | Part B surcharge | Part D surcharge |
|---|---|---|---|
| $116,000 or less | $224.50 | $0.00 | $0.00 |
| Over $116,000 to under $384,000 | $718.30 | $493.80 | $93.60 |
| $384,000 or more | $763.30 | $538.80 | $102.20 |
Part D surcharge is added to your plan premium. Married-separate tiers shown apply if you lived with your spouse during the tax year.
Calculate my surchargeQuestions, answered.
Are the 2028 Medicare amounts official?
No. These are Trustees-based premium projections and explicit IRMAA planning assumptions, with no claim of a final CMS announcement.
Which income counts for IRMAA?
Modified adjusted gross income is ordinarily adjusted gross income plus tax-exempt interest from two tax years earlier. Certain changes can justify a new determination from SSA.
Will a married couple pay one premium?
Medicare costs are generally per enrolled person. Joint filing uses the joint income thresholds, but each spouse can owe a premium and surcharge.
Does the Part D base amount equal my plan premium?
No. It is a national benchmark. Your chosen plan premium and any Part D income-related surcharge are separate.
Can retirement reduce an IRMAA assessment?
Work stoppage or reduction can be a qualifying life-changing event. Follow SSA-44 and SSA’s evidence rules; a lower entered calculator income does not itself change the assessment.
